IntroductionYounger age at diagnosis is a risk factor for poor health-related quality of life in long-term breast cancer survivors. However, few studies have specifically addressed HRQOL in young adults with breast cancer, nor have early changes in HRQOL been fully characterized.MethodsEligible female patients with breast cancer were identified through our local cancer center. To establish HRQOL, patients completed the Functional Assessment of Cancer Therapy-Breast around diagnosis and 12 months later. Sociodemographic factors, genetic susceptibility to cancer, tumor- and treatment-related factors, and (...) comorbidities were abstracted from medical records and the local oncology registry. Mixed-effects models were used to identify changes in FACT-B scores during the first year of treatment and to determine whether any demographic/treatment-related factors modulated changes in scores.ResultsHealth-related quality of life in young patients with breast cancer was within normal limits at baseline, with a FACT-B overall well-being score of 108.5. Participants reported slight improvements over a 12-month period: FACT-B overall well-being scores increased 6.6 points, functional well-being improved 3.0 points, emotional well-being improved 1.9 points, and physical well-being improved 1.5 points, on average. Participants with anxiety/depression at baseline reported greater improvements in FACT-B overall well-being and functional well-being than participants who did not have anxiety/depression at baseline. Marital status, reconstructive surgery, and baseline clinical staging were also significantly associated with changes in aspects of HRQOL, although their impact on change was relatively minimal.ConclusionYoung women with breast cancer do not report HRQOL concerns during the first year of treatment. Improvements in HRQOL during the first year of treatment may be attributable to a sense of relief that the cancer is being treated, which, in the short run, may outweigh the negative late effects of treatment. The Malabo Declaration places the transformation of agriculture and food systems at the centre of regional and national policy priorities across Africa. Transformative change in the way that food is produced, processed and consumed is seen as not only necessary for addressing the complex challenges of food security and poverty alleviation, but also as a driver of new employment opportunities and economic development. As pointed out within the recent UN Food Systems Summit, essential elements of food system transformations include digital (...) transitions and the empowerment of women and youth. However, there are few empirical examples demonstrating how these agendas come together to affect food system change. Here we focus on an enterprising group of young farmers referred to as Malawi’s _new achikumbe elite_, who are urban based, educated and engaging in agriculture on a commercial basis. The aim is to characterise this emergent group of agriculturalists and to understand the role that they have within the transformation of Malawi-s agricultural sector. We explore how digital platforms are supporting the emergence of this new category of farmer and positioning young people as agents of change in food systems transformation. Based on interviews and ethnographic research with 32 young farmers between 2018 and 2022 combined with interviews with representatives of service providers and agricultural organisations, we argue that this group is characterised by a higher level of education, self-dependency and use of digital platforms, enabling them to adapt their context to sourcing production resources and engaging in commercial agriculture. We present evidence that digital platforms are supporting the new achikumbe elite (NAE) to engage flexibly with new commercial markets, contracts and access a wider range of training and advice. However, while digital platforms can offer more equitable access to information and market opportunities, they also represent potential avenues for food system transformations that are inequitable. As such, we argue that there is need for digital technologies to mitigate against potential inequalities. (shrink)

Beginning with a review of Michael Pollan's latest book about the renaissance of research into the use of psychedelics to treat addiction, depression, and end-of-life anxiety, this essay considers wisdom and insight that might be gained by examining the psychedelic practices of primitive people. Pollan finds that almost all who begin using psychedelics to treat the ill eventually come to the conclusion that they should be made available for the broader purpose of 'the betterment of well people'. By considering both (...) scientific and spiritual perspectives on these compounds, the book offers insight into the toxicity not only of our mental environment, but at the same time our relationship to fellow humanity and Mother Nature. However, Pollan does not consider what is perhaps the greatest source of wisdom regarding the pharmacological potential of psychedelics: primitive societies who have long incorporated these compounds into their ways of being. By casting anthropological accounts of primitive psychedelic practices against those of contemporary research, this essay seeks to deepen enquiry into the pathologies of modern personhood as they bear upon the ecological crisis, together with the potential value of entheogens for understanding and responding to these problems. On various principles of economics in an Islamic society.

I am pleased by the degree of agreement about reasons between the three of us, which is much greater than I might have guessed. I have no objection whatever to the project of giving the kind of psychological description of deliberation about reasons that Gibbard proposes. I agree that “weighing X in favor of A isn’t mysterious,” but I do confess to some doubt about how a psychological description of this process of weighing “explains, indirectly, X’s counting in favor of (...) A in such circumstances.” If there is any disagreement between Gibbard and me on this point, I think it is over the degree to which such an explanation is to be expected. (shrink)

This paper explores the influence of social categories on the perceived trade-off between a relatively bad but equal distribution of resources between two parties and a profit maximizing yet unequal one. Studies 1 and 2 showed that people prefer to maximize profits when interacting within their social category, but chose not to maximize individual and joint profits when interacting across social categories. Study 3 demonstrated that outside observers, who were not members of the focal social categories, also were less likely (...) to maximize profits when resources were distributed across social category lines. Study 4 showed that the transaction utility of maximizing profits required greater compensation when resources were distributed across, in contrast to within social categories. We discuss the ethical implications of these decision making biases in the context of organizations. (shrink)
